The issue with making Fear and Hunger into a mafia game is that you cannot properly inject the gameplay loop of fear and hunger into a mafia format
something about raspberry filling and chocolate melting etc etc

Other games with heavy immersive sim elements work far better

Although you would need to explicitly label the game as immersive sim-y
Eevee’s Zombie Apocalypse game shows that such a method works even if not perfectly
