Yes, it’s not realistic. It’s literally a cartoon. The neck is twisted to show off the choker which would otherwise be fairly distorted. There are likely many different accessories one can place on the neck in the picrew creator and for the purposes of highlighting choices the person made, visual clarity was emphasized over realness. This is incredibly normal stuff.
